Calling local GPs to give feedback on discharge summaries

Dr Chris Mostert (Physician), Dr. Zayn Al-Timimi (JMO), and Dr. Isobel Waugh (JMO) are conducting an informal survey of local general practitioners regarding the general satisfaction with and efficacy of discharge summaries from Coffs Harbour Health Campus.

They are interested in finding out more about your experiences with their discharge summaries – what works well, what may not be necessary, and what may be improved.

They are mindful of the often limited ability to follow up patients once discharged, and wish to ensure patients are being handed over to their primary care physician with a sufficient level of information and clarity of plans for ongoing management to ensure we are achieving the best possible outcomes for our patients.

This is an informal survey being conducted for quality improvement purposes. It is intended that the data obtained will help to optimise discharge summaries based on your feedback and disseminate findings at a departmental level.
