This session provides an opportunity for hospital specialists and primary healthcare clinicians to learn from each other, improve communication, and work together to enhance referral pathways and patient outcomes across our region.

Our presenters will share how their teams support local patients and families, while attendees will have the opportunity to discuss challenges, share insights from primary care, and explore ways to deliver more connected, coordinated care.
