Join the LUMOS Project

The LUMOS project securely links encoded data from general practices to other health data in NSW, for example hospital and emergency department data. This is done under strict data governance processes in a partnership between Primary Health Networks and the NSW Ministry of Health.

When aggregated, LUMOS data demonstrates the patient journey through primary and secondary care.

LUMOS aims to:

• Generate information and insights around the patient journey across health care services.
• Provide GPs with meaningful information about their patients that can help to improve care.
• Support data-driven quality improvement in general practice.
• Provide an evidence base to support areas of future investment in primary care.
• Support greater collaboration in health to ensure the right care in the right place at the right time.

• The data custodian for your practice consents to the collection of de-identified data. Download LUMOS GP information, FAQS and consent form.
• The data is linked twice yearly and the aggregated data is returned to the practice in a dashboard report. This is an automated process that occurs without practice time or intervention.
• You can use the data to plan evidence-based improvements.
• There are opportunities to become a GP Clinical Champion and to participate on statewide committees over the life of the project.
