Supporting primary care across the North Coast
Primary care is often the first point of contact people have with the health system and plays an essential role in keeping our communities healthy.
Healthy North Coast works with general practice, ACCHOs, allied health, pharmacy, nursing and other primary care professionals and services across the North Coast to strengthen local primary care and improve access to coordinated, person-centred care.
We support:
• general practitioners and general practice teams
• practice managers and administrative teams
• nurses and nurse practitioners
• allied health professionals
• pharmacists
• Aboriginal Community Controlled Health Organisations and Aboriginal health professionals
• residential aged care providers and clinicians
• other primary and community care professionals.
Primary Care (PCI) Impact Grants.
Primary Care Impact Grants support general practices and primary care providers to develop and implement initiatives that respond to local health needs and create meaningful improvements for patients and communities.
The grants provide an opportunity to strengthen models of care, improve access and patient experience, build practice capability and test new approaches that can contribute to a stronger primary care system across the North Coast.
Sustainability and Viability Initiative (SAVI)
SAVI supports primary care teams with access to information and insights that can help inform planning, quality improvement and better patient care.
By making relevant information easier to understand and use, SAVI can help practices identify opportunities, understand their patient population and make informed decisions about where improvements can have the greatest impact.

Our approach to an integrated heath system
Improving health outcomes requires organisations across the health system to work together.
Healthy North Coast connects primary care with hospitals, Aboriginal Community Controlled Health Organisations, commissioned services, community organisations and other partners to improve coordination and strengthen pathways between services.
Through shared planning, commissioning, workforce development, data and insights, and locally designed models of care, we are working towards a more integrated health system where people experience care as connected — regardless of which part of the system provides it.

Our Regional Managers work directly with general practices and primary care teams across the North Coast, providing a local point of contact with Healthy North Coast.
They can help connect you with relevant programs, resources and opportunities, provide support with local primary care initiatives, and help you navigate the services and support available through Healthy North Coast.
