The Link-Me trial is a trial funded by the Department of Health and is working closely with the University of Melbourne which seeks to identify greater avenues for complex mental health care in general practice.
The trial aims to address this gap and determine whether a specific role could assist patients in their care planning at the GP. The trial is a randomized control trial.
Participants identified as being likely to experience severe and complex mental health problems will receive assistance from a practice-based mental health care navigator to develop an individual care package.
The care navigator will work with the patient, the GP and other providers to develop a care plan and support the patient through their care and ensure that elements of the care package are connected. The trial is focused on the Mid North Coast and will run for 18months.
